Supporting People to Return Home
from Secure Care or Out of Area Placements

This theme draws together the NDT’s knowledge and experience in working with people in secure settings and out of area placements – bringing together the essential ingredients in local communities to support people to return home. This includes: risk assessment, management and processes, person-centered approaches and individualised services, commissioning & care management.

Approaches


Local analysis
By working with a cross-section of stakeholders (commissioners of local learning disability and secure services, clinicians, practitioners, person-centered planners, independent providers, National Offender Management Services) we can build a profile of respective roles and partnerships to inform an accountability map for these individuals. We can change the way information is shared, our involvement and partnerships, in order to shift the pattern of exporting people miles away from home.


Mapping the process
Mapping the journey – finding out who is where and why? What are each individual’s needs and risks and why were they exported from local services? A service and system’s ‘health-check’ – what state of readiness are your key staff, how robust are your information systems, your risk management and person-centred approaches to care? We would work with you in gathering local intelligence about your community’s capacity to support these individuals and what would need to change to ensure that the individual and their community could support each other more effectively.

Whole systems approach

Supporting people to return home – a whole systems approach. Locally tailored consultancy, service develop-ment, training with team and partnership support. This could include elements of all of the following:

Agreeing the vision across services.
Finding out what works in other communities and the essential ingredients for success.
Person-centred approaches and how that informs service planning.
Staff training and development in person-centred approaches, risk management, commissioning for outcomes.
Commissioning for groups and with individuals.
Building capacity within services and within communities.
Commissioning through crisis. Contingency arrangements.
Effective team and partnership working.
